Request for Proposal: Local University Support for Baseline Data Collection in USAID Supported Districts in ‘IMPROVING ACCESS TO AND QUALITY OF PRIMARY HEALTH CARE (PHC) SERVICES IN INDONESIA’
Background
In April 2022, the Indonesia Ministry of Health (MOH) launched its health transformation agenda, identifying six areas of work (pillars) where the MOH intends to focus going forward given the population’s changing health burden status and the need for acceleration in particular health areas to meet the country’s minimal service standards (SPMs). Primary care transformation, the first pillar, represents the government’s plans to take a more preventive and promotive approach to health, increasing access to integrated services across the life cycle by bringing quality primary health services closer to community level. The Integrasi Layanan Primer (ILP) model calls for the introduction of a more standardized continuum of care approach that focuses on prevention and promotion, connecting community health workers (and home visits) to the Posyandu, then to the Pustu prima (a “new” level of care that consolidates multiple community health structures), and then to the Puskesmas, ensuring referrals to higher levels of care as necessary.
USAID/Indonesia is planning to support the MOH in replicating the I-PHC model in 79 districts. USAID-MOMENTUM (MCGL) will support PHC strengthening through technical assistance to 44 districts across five provinces (Banten, East Java, North Sumatra, NTT and South Sulawesi). Other USAID partners will support PHC in an additional 35 districts in the four provinces (the same as MCGL except for NTT).
USAID/Indonesia plan to support ILP in a total of 101 districts. Overall baseline data collections will happen in two phases:
- PHASE 1 – April – May 2024
In house baseline data collection by MOMENTUM team in 44 districts (the initial 44 districts that receives full period technical assistance)
- PHASE 2 – May 2024
Baseline data collection by university partner in 35 districts:
MENTARI-PHC: 15 districts, 45 Puskesmas
INTEGRASI: 20 districts, 52 Puskesmas
PHC baseline data collection instruments are new. Having the two phases of baseline data collections will allow MOMENTUM team and university partners to apply any lessons from phase 1 (44 districts) in the phase 2 (35 districts).
MCGL will assess the current status of PHC 1 using the USAID Primary Impact Measurement for Action Framework and other indicators included in the MCGL Monitoring, Evaluation and Learning (MEL) Plan in the 44 districts supported by USAID under MCGL.
Objectives
The objective of the SoW for the local universities is:
To gather baseline data for the agreed PHC indicators in 35 districts (USAID-supported intervention districts through two local organizations)
Scope of Works
It is anticipated that this activity is to be conducted by a network of universities involving local universities located in the sampled districts. The leading university/institution will have to ensure the quality of the work done by local universities involved.
- In collaboration with MCGL, when necessary, adapt the study protocols, tools and data collection guidelines
- When necessary, secure ethical approval for the study as well as permission from the study sites/facilities
- Manage any sub-awards to ensure that deliverables are submitted on time and of high quality
- Conduct centralized data analysis of the baseline.
- Communicate with MCGL team routinely to update the progress of the work as well as discuss any emerging issues
- Write report of the baseline
It should be noted that the scope of work can be extended to include routine monitoring in the 97 districts as continuation of the baseline (e.g. six monthly). Further discussion will be held after the completion of this scope.
Sites
The activity will be done in the four provinces of USAID-MOMENTUM project (i.e. North Sumatera, Banten, East Java, and South Sulawesi). The number of districts covered for the baseline is 35.
Methodology
Indicators. The indicators are comprised of standardized global PI indicators at input, process and output levels (annex 1).
Methods. Data will be collected from a mix of client, facility, and district levels as well as analysis of existing HMIS data.
The global USAID Primary Impact Measurement for Action Framework has standardized measurement tools. These will be used and include the following:

Design. Descriptive analysis will be conducted.

Ethical considerations
MOMENTUM is governed by the JHU School of Public Health IRB. A non-research determination will be pursued as the assessment will be conducted to help implement the government policies, but the actual determination will depend on the methods. Local ethical approval will be sought as needed.
