Health Financing Assessment of the Impact of USAID Funding and Domestic Budget Reduction on the MNCH and Immunization Services in Indonesia

The Clinton Health Access Initiative, Inc. (CHAI) is a global health organization committed to its mission of saving lives and improving health outcomes in low and middle-income countries by enabling governments and the private sector to strengthen and sustain quality health systems.
CHAI was founded in 2002 in response to the HIV/AIDS epidemic to dramatically reduce the price of life-saving drugs and increase access to these medicines in the countries with the highest burden of the disease. Over the following two decades, CHAI has expanded its focus. Today, along with HIV, we work in conjunction with our partners to prevent and treat infectious diseases such as COVID-19, malaria, tuberculosis, and hepatitis. Our work has also expanded into cancer, diabetes, hypertension, and other non-communicable diseases, and we work to accelerate the rollout of lifesaving vaccines, reduce maternal and child mortality, combat chronic malnutrition, and increase access to assistive technology. We are investing in horizontal approaches to strengthen health systems through programs in human resources for health, digital health, and health financing. Program Background & Position Overview
CHAI’s Health System Strengthening group works with governments to build stronger, more resilient, and more equitable health systems, ensuring that everyone, especially the most vulnerable, can access high-quality primary health care. We support the Ministry of Health to increase and optimize health spending, strengthen national systems (e.g., workforce, digital infrastructure, supply chains), and implement cross-cutting reforms that drive long-term improvements in service delivery and health outcomes.
The government today faces growing pressure to do more with less. Economic constraints, inflation, and rising health needs are straining the budget, while donor funding – including foreign assistance from major bilateral partners – is stagnating or declining in many countries. These challenges are creating serious gaps in service coverage and threatening hard-won gains in health. In response to these transitions, Clinton Health Access Initiatives (CHAI), with support from the Gates Foundation, is launching a high-priority initiative to support the government in responding to the growing pressures of shrinking aid budgets, including recent USG funding reductions and broader global transitions in donor support. To support this effort, CHAI seeks an individual consultant or consulting team to conduct an impact assessment study of the USAID funding and domestic budget reduction on the MNCH and immunization services, aiming to strengthen evidence and advocacy to inform prioritization and technical efficiency of MNCH and immunization financing.
Objectives
The overall objective of this consultancy is to identify the critical budget gaps and their influence on the MNCH and immunization service outcomes, providing actionable recommendations for strategic prioritization and optimized resource allocation. Specifically, the study aims to:
- Quantify funding gaps and assess impact
- Conduct a comprehensive study on the budget allocation (resource mapping) and expenditure tracking of maternal, newborn, and child health (MNCH) and immunization services sourced from USAID donor grant, regional budget (APBD) and special allocation fund or health operational assistance sourced from state budget transfer (APBN) within targeted districts from the period of 2023-2024 (baseline) and after USAID funding cuts in 2025-2026
- Employ impact evaluation to identify the effect of the USAID budget cuts linked to the MNCH and immunization services coverage outcomes
- Identify the funding gaps, barriers, enablers, and community or stakeholder perspectives on the impact of budget cuts
- Assess the sustainability of activities that were previously funded by a USAID grant and the adaptation strategy utilized by the district health offices and primary health facilities after the USAID funding cuts
- Strengthened evidence to inform prioritization, technical efficiency, and sustainability
- Develop evidence-based recommendations to support prioritization and enhance the efficiency, effectiveness, and sustainability of the MNCH and immunization programs throughout the transition from USAID support
- Develop strategic recommendations for cross-program integration and resource mobilization
- Highlight strategic areas for cross-program integration for MNCH and immunization to optimize resource management and maintain health outcomes performance
- Map out the potential domestic resources landscape to secure sustainable program support, i.e. village fund, BLUD fund, and CSR
Scope of Work (SoW)
The consultant shall complete the following activities:
- Develop a robust and comprehensive technical proposal outlining methodology and a detailed budget plan
- Design data collection tools, including resource mapping of USAID, special allocation fund (BOK), and regional budget, funding gap analysis, expenditure analysis, program outcomes indicators, and data analysis techniques to identify the impact.
- Develop the study site selection criteria across 2 districts of previous USAID-assisted districts within at least 2 out of 6 provinces in East Nusa Tenggara, East Java, DKI Jakarta, Banten, North Sumatra, and South Sulawesi that can represent the overall USAID cut & domestic resource impacts, ensure representation across program coverage and fiscal capacity.
- Develop and submit the study protocol to local Ethics Committees and the Ministry of Health for administrative approval, complete the requirements, and ensure the protocol is finalized and approved
- Conduct assessment/mapping according to the agreed design and methodology
- Perform responsible data management, which involves maintaining data quality and appropriateness, ensuring safety and confidentiality throughout the process, from data collection, storage, cleaning, analysis, and preservation.
- Analyse and synthetize data to produce key findings on the impact of the USAID funding cuts on the immunization and MNCH services at selected district and primary health facilities levels
- Develop a comprehensive report with compelling data visualizations, lessons learned, and key recommendations
- Conduct regular coordination meetings with CHAI to update progress and preliminary findings
- Conduct project implementation and financial management and reporting, in compliance with CHAI SOP, guidelines, and service agreements
- Produce final narrative report and slide presentation (in English and Bahasa Indonesia), summarizing the key findings from the analysis of the impact of USAID funding cuts and domestic budget allocation on the MNCH and immunization services outcomes, along with evidence-based recommendations for program improvement
- Evaluation of the impact of USAID funding cuts on the MNCH and immunization services outcomes in the selected USAID-assisted districts
- Evidence-based advocacy materials and recommendations to support prioritization and enhance the efficiency, effectiveness, and sustainability of the MNCH and immunization programs financing amid the fiscal constraints
- Highlight strategic areas for cross-program integration to optimize resource management and maintain health outcomes performance
Qualifications
CHAI invites individual/team to participate in this project with detailed requirements as follows:
- At least one (1) research team member(s) with an advanced degree in health economics, public health, or other relevant disciplines.
- Knowledge and experience in health system strengthening, health financing, health economics, with a specialized focus on maternal, newborn, and child health, and immunization programs
- Demonstrated proficiency in designing and executing robust evaluation studies and analytical frameworks
- Advanced analytical skills with experience supporting policy development focused on strategic prioritization and optimized resource allocation
- In-depth familiarity with the Indonesian health policy landscape, specifically regarding health financing reforms and national MNCH and immunization strategies

- Methodology, sample selection, and data analysis plan
- Implementation plan (including team division/allocation) and project timeline
- Anticipated risks and mitigation plans
- Reference list/bibliography
- Annex 1. Annotated budget (in Excel format) with breakdown for honoraria (unit cost/rate must be in accordance with the recent SBU regulation from the Ministry of Finance), activities, and other anticipated costs
- Annex 2. CVs of key personnel (Principal Investigator/PI, research assistants, and field coordinators) which showcase the profiles corresponding to the required qualification and list of publications. All submitted CVs must be merged in one (1) PDF file.
- Annex 3. 1 sample of previous work (published paper or report, in PDF format)
Project Timelines
The total expected duration of the assignment is around 4 months from March 1 to June 30, 2026. All reports must be finalized and submitted within this period to the CHAI Project Manager, Niken Widyastuti. The time frame for activities to complete the deliverables is based on the following:
- Progress update on the data collection plan, research framework, qualitative and/or quantitative instruments, DHO coordination, and ethical clearance submitted – Deadline March 31, 2026
- First draft report and presentation slides on the impact assessment of the USAID funding termination on the MNCH and immunization services outcomes – Deadline March 31, 2026
- Consolidated final report submission, in both English and Bahasa Indonesia:

Budget and Disbursement
The projected budget for this project totals IDR 130,000,000. This amount includes the consultant fee, expenses related to activities necessary to deliver the objectives during the study, and is inclusive of all applicable income tax expenses. Payment will be disbursed by deliverables. The costs of the dissemination meeting will be covered by CHAI Indonesia.
